About the ACH Inventory

The Accountable Communities for Health (ACH) Inventory is maintained by the Funders Forum and is a tool for learning more about the current landscape of ACH and ACH-like initiatives across the nation. Implementation of the accountable health model varies considerably depending on the initiative’s goals, priorities, and unique needs of the communities in which they operate. Nevertheless, all ACHs and ACH-like efforts share several common elements. Importantly, these efforts can serve as one vehicle to center community voice in governance and resource allocation within communities, many of which have been impacted by structural racism and other patterns of divestment.

As of June 2024, the inventory included 185 sites across 33 states and Washington D.C. This inventory is a living document that incorporates publicly available data and information from site representatives.
