The ACH Inventory features information on ACHs and ACH-like initiatives across the United States, with categories including the state, name, backbone organization, key focus areas, types of health-related social needs interventions, and website. Click “view more” on the right side of the table to learn about each initiative’s funding sources, partners, approaches to community governance and engagement, specific population focuses, and outcomes.
You may also utilize the search function to explore specific topics in which you are interested. For example, if you are interested in initiatives that address specific health-related social needs, try search terms such as education, economic stability, family and social support, food, housing, interpersonal safety, physical activity, transportation, and/or utilities.
