Logic Model
The Funders Forum developed the Logic Model in 2017 to capture the process by which a multi-sector collaborative becomes an Accountable Community for Health (ACH).
This Logic Model was informed by existing logic models from federal, state, local, and private philanthropic accountable health initiatives. The logic model is not prescriptive – rather, by embracing a nonlinear theory of change, the gears are meant to show that different communities lie on various ends of spectrum and may move forward or backwards along each spectrum as they develop. The gears show the interconnected ways that enabling conditions, environmental context, systems design and development, and short- and long-term outcomes interact to drive continued learning in accountable health initiatives.
