Inventory of Accountable Communities for Health

The Funders Forum of Accountable Health is working to catalog existing Accountable Communities for Health (ACH) and ACH-like efforts nationwide. Accountable Communities for Health can go by many names, including Accountable Care Communities, Coordinated Care Organizations, or Accountable Health Communities.  These initiatives are focused on community-based strategies for integrating the health care and social needs of individuals and differ from Accountable Care Organizations (ACOs), which are health care provider-driven initiatives.

This inventory reflects publicly available data and outreach to several ACH leaders.  We welcome feedback on the sites and descriptions listed below as well as information about any other sites that should be included.  Please email us at [email protected].
